Christian Formation Grant Criteria:

For programs in catechetical/pastoral ministries, the Catholic Community Foundation seeks to fund programs that nurture the spiritual growth of the people of the Diocese of Phoenix. Special consideration will be given to parish-based initiatives that utilize established diocesan programs.

General Grant Requirements:

1. Applicants must be a registered 501(c)(3) charity in Arizona.
2. Grant recipients or an agency representative must be present at the Grants Reception in the Fall in order to receive the grant.
3. Grant recipients must submit a letter of acknowledgement for grant funds received to the Catholic Community Foundation to be shared with its Board of Directors, Grants Committee, and others.
4. Grant recipients must consent to signing a photo/video release.

Christian Formation Grant-Specific Requirements:

1. Applicants must provide a current list of the parish’s Pastoral Council and/or Finance Council, including addresses, with their applications.
2. Applicants must submit a Program/Project budget with their applications.
3. If grant funds are used for an educational course, the parish must forward a Certificate of Completion for each student funded, or letter naming the students who completed the coursework, to the Catholic Community Foundation.

Please Note For All Grants:

1. Requests for funding of salaries and capital improvements do not qualify for grant monies.
2. Due to the finite amount of grant monies available, not all proposals can be funded.
3. Funding in the past does not guarantee continued funding.
4. Lack of funding in the past does not indicate that funding will not be possible in future years.

HOW TO APPLY:

1. Grant applications are in MS Word format, and are designed to be filled out on your computer, then printed for signature and submission. Alternatively, you may save the application on your computer hard drive and open it in MS Word to fill in at a later time.
2. Applications must be typed. Handwritten applications will not be accepted.
3. Applications must be signed by the applicant AND the Pastor or Program Supervisor.
4. Completed application packets may be submitted in one of the following ways:
1. Mail completed and signed application and required documentation, postmarked by November 15th to:

2. Fax completed and signed application and required documentation, by November 15th, to the Catholic Community Foundation at 602-354-2423
3. Email scanned copy of completed and signed application and required documentation, by November 15th, to the Catholic Community Foundation at info@ccfphx.org
